Each patient should have the freedom to make decisions regarding life-sustaining treatments, advance care planning, and how they wish to carry out their final days.
St. John’s compassionate care team will have end-of-life conversations with the patient and their loved ones to establish a plan of care that is tailored to their needs and wishes. The team will focus on how the patient wants to live their life according to what is meaningful to them and their family. Hospice care is intended to improve the quality of life by involving the patient and their family in setting goals and participating in the decision-making process.
